What is Primary Care?
Primary Care is the foundation of a person's healthcare journey, providing comprehensive and continuous medical care for individuals of all ages. Our primary care providers serve as a first point of contact for patients seeking preventive care, treatment for common illnesses and injuries, and management of chronic conditions. Services provided by primary care include routine check-ups and counseling on maintaining a healthy lifestyle. Our Primary care providers also coordinate and refer patients to specialists when necessary, ensuring a holistic approach to healthcare tailored to each individual's unique needs. In essence, primary care focuses on building relationships with patients, promoting overall health and wellness, and serving as a trusted partner in managing and improving one's health.
In-Office
Here at Restorative Health, we strive to provide a comfortable and welcoming environment for all of our patients. We are dedicated to giving each patient the personalized attention they deserve. Whether we are seeing a patient for a routine check-up or a specialized treatment, we are here to support you every step of the way.
Telemedicine
For the convenience of our patients, we offer telemedicine visits hosted through Google Meets. Many patients can receive expert medical advice and support from our team from the comfort of their own homes. We can refill prescriptions, give personalized consultations, and answer any of your questions via telemedicine.
Primary Care Appointments
Simple Visit | $75
This visit covers single prescriptions or a simple or single medical concern.
Moderate Visit | $130
This visit is best for patients with several ongoing medical conditions, a single new diagnosis, treatment evaluations, two to four prescription refills, or evaluations for new prescriptions.
Complex Visit | $230
The complex patient visit is for patients with multiple complex medical conditions or numerous prescriptions needing refills or adjustments.
Refill visits are necessary for ongoing medication management, typically scheduled anywhere from 1 to 12 months depending on the medication and the patient's condition.
Additionally, any change in medication dose or route, such as switching from a nasal spray to a troche, requires a prior visit.

Pain Management
Pain Management Visit | $130 - $230
A pain management visit with our medical providers typically involves a comprehensive evaluation of a patient’s pain history, symptoms, prescriptions, and previous treatments. This includes an in-depth discussion about the nature of one’s pain and the impact it has on daily activities and quality of life. Based on the assessment, the provider will develop a personalized care plan. This may include medications, interventional procedures (e.g. IV or IM Ketamine), life changes, and/or referral to other specialists. For each pain management patient, our goal is to alleviate pain, improve functionality, and enhance overall quality of life.
Documentation of previous medical records/medications will be required at this visit.
Emotional Support Animal Recommendations
ESA Letter Visit | $75
An emotional support animal (ESA) is a pet that provides comfort and companionship to individuals suffering from mental health conditions such as anxiety, depression, or post-traumatic stress disorder (PTSD). Unlike service animals, ESAs do not require specific training to perform tasks but instead offer therapeutic benefits through their presence and the emotional bond they share with their owners. Our medical providers at Restorative Health are able to prescribe an ESA to qualified patients, with the goal of enhancing their emotional well-being and improving their overall quality of life.
Sick Visits
Sick Visit | $75
Sick visits are quick appointments with concern for bacterial/viral infections (e.g. sore throat, sinusitis, bronchitis, etc.). During the visit, the medical provider performs a physical examination to assess the patient’s condition and medical history. If the provider determines antibiotics are necessary, the appropriate medication and dosage will be prescribed, along with instructions on how to take it and potential side effects.
FMLA, Worker's comp., and disability Paperwork
FMLA, Workman's Comp., Disability Paperwork Visit | $230+
At Restorative Health, a FMLA visit (Family and Medical Leave Act) or a Worker’s Compensation visit involve meeting with a healthcare provider to discuss medical leave and/or work-related injury. Our providers will need proper documentation of any claims. During the visit, the provider will conduct a thorough evaluation of a patient’s medical condition, which may include a review of medical history, a physical examination, and any relevant diagnostic tests. The goal of the visit is to assess the illness/injury, determine necessary leave time, and develop a treatment plan. After the visit, our providers will complete all required forms, detailing diagnosis, treatment plan, and estimated duration of leave and/or work restrictions. We ensure that each patient receives appropriate care while facilitating the legal and administrative processes needed for medical leave and/or compensation benefits.
Birth Control
Birth Control Visit | $75
A birth control visit involves our medical provider discussing contraceptive options to find the most suitable method for the needs and lifestyle of the patient. During this visit, the provider will conduct a comprehensive evaluation of the patient’s medical history, including menstrual cycles, sexual activity, and any relevant health conditions or medications. A physical examination and lab draw may accompany this visit. Various birth control methods including pills, patches, vaginal rings, or the depo provera shot are discussed, along with their benefits, potential side effects, and proper usage. The goal is to provide personalized contraceptive advice, ensure the patient’s understanding, and address any questions or concerns relevant to reproductive health.
Hair Restoration
Hair Restoration Visit | $130
A hair restoration visit with our clinic begins with a detailed consultation, where the provider reviews the patient’s medical history, hair loss pattern, and overall health. We may order a blood test to identify any underlying conditions contributing to hair loss. The provider will discuss medication options with the patient (Finasteride or Dutasteride). Finally, the providers will work with the patient to develop a personalized treatment plan, based on the patient’s individual needs and goals. The visit usually concludes with a discussion about expected outcomes, potential side effects, and follow-up appointments for monitoring hair growth progress.

Palliative Care
Palliative Care Visit | $75 - $230
Our palliative care plans are meant to provide comfort and relief from life-limiting conditions and terminal illnesses. We focus on improving the quality of life of our patients through comprehensive symptom management and emotional support. This form of medical care provides an extra layer of support, helping individuals manage their illness more effectively and enabling them to remain as functional and comfortable as possible. Our medical providers focus on addressing their patient’s physical, emotional, and psychological needs to aid patients and their families in finding moments of peace and stability during challenging times.
Sports Physical Exam
Sports Physical Exam | $130
A sports physical exam, often required for participation in athletic activities, involves a comprehensive examination of a patient’s overall health and physical fitness. The provider will review the patient’s medical history, past injuries, illnesses, and any medications they are taking. A physical examination, checking of vital signs, and examination of the patient’s general physical condition will be conducted during this visit. Specific attention is given to the musculoskeletal system, assessment of strength, flexibility, and joint health. The provider may also evaluate the cardiovascular health to ensure there are no underlying issues that could pose risks during sports activities. Our goal is to identify any conditions that might affect the athlete’s performance or increase risk of injury by providing recommendations or treatments necessary to ensure safe participation in sports activities.
LDS Mission Physical Exam
LDS Mission Physical Exam | $130
A LDS mission physical exam involves a thorough health evaluation required for individuals preparing to serve a mission for The Church of Jesus Christ of Latter-day Saints. This visit will include a detailed review of the patient’s medical history, immunization records, and any chronic conditions or medications. The provider will conduct a comprehensive examination of physical health, assessing vitals, and overall physical health. Additional tests, such as lab work or screenings, may be ordered as needed. The goal of this visit is to ensure the patient is in good health and capable of handling the physical and emotional challenges of missionary service.
