All new ketamine patients are required to have 30 minute discovery consultation to determine whether ketamine infusions are an appropriate medical treatment.
Ketamine IM Infusion | $215
Ketamine is administered intramuscular. Typical dosing for infusions is 50 mg and 125 mg based on patient weight, patient feedback and health goals
Ketamine IV Drip Infusion | $250
Ketamine is administered intravenously over the course of 45 minutes

The ketamine guide will be with you the entire duration of the infusion.

Simple Patient Visit 15 min | $65
A simple patient visit is ideal for patients with one prescription refill
Moderate Complex Patient Visit 30 min | $130
All new scheduled drug prescriptions require a 30-minute appointment. Applicable to both new and established patients. This visit is best suited for patients that have two medical conditions or prescriptions that are ongoing.
Complex Patient Visit 60 min | $205 (Recommended for all new patients)
Complex patient visits are the most in depth visit offered with a comprehensive look into all aspects of a patient's health; including review of past and current health issues, medications, treatments, diet, environmental and lifestyle factors, and develop in individual health plan based on goals
This visit is ideal for patients with three or more issues to cover during the medical evaluation
Bio Identical Hormone Therapy
Bio Identical Hormone Consultation | $130
Bio Identical Hormone Therapy is intended to relieve symptoms of hormone imbalance, improve cardiovascular health, bone production, and help the bodies inherent system of growth and repair. Testosterone and estrogen imbalances can contribute to issues like hot flashes, night sweats, low sex drive, sleeplessness, and mood changes.

Utah medical card requires two six month follow up and then annual follow up appointments to renew.
Compassionate Use Board Approval | $105
Utah law requires that for patients under the age of 21 or patients that don't have a qualifying diagnosis we must submit additional documentation for the compassionate use board. This is an extra step in addition to the medical cannabis appointment.

30 minute visit with a medical provider detailing interventions, goals and additional information about the program.
Initial Month of Medication Administration | $270
The remaining balance of the monthly cost of the program will be charged. This includes 4 weeks of injections of Semaglutide, Super MIC, and B12.
Monthly Cost | $400
Monthly follow up appointments Include 15 minute monthly visit with a medical provider to track progress against health goals, monitor weight loss and potential side-effects. The program is month to month, charges occur as patients decide to continue with the program. Fees vary from $400 - $450 depending on dosage.
Semaglutide, sold under the brand names Ozempic, Wegovy and Rybelsus, is a modified molecule our bodies make, called a GLP-1 (gut hormone). It’s a protein made in the intestine and results in a decrease in blood sugars and appetite. A small change in the molecule allows the drug to last weeks in our bodies rather than the nature version our bodies make, which lasts minutes. Originally used for diabetes and sold under the name Ozempic, SemaGlutide is a higher dose of the same drug for weight loss.

This care serves as an extra layer of support to help you manage your illness and remain independent at home. If you are receiving care in an assisted living we can work alongside your healthcare team to provide palliative care in that setting

Ideal for individuals suffering from migraines. Similar IV cocktail administered in well managed hospital emergency rooms.
